    Claim Procedure

    In the case of hospitalisation, the policyholder will have to follow the steps mentioned below to register a claim. In the case of planned hospitalisation, the insured person has to intimate Max Bupa at least 72 hours (3 days) before being admitted in any of the network hospitals of the insurer.

    • The insured person has to submit his/her Max Bupa insurance card along with proof of identification - Passport, Driver’s license, Voter ID card, PAN, etc. - at the network hospital desk.
    • The hospital will then submit a pre-authorisation form to Max Bupa with details of the treatment.
    • The insurer will then review the case and if the particular treatment can be covered, the decision to offer coverage for hospitalisation will reach the hospital within 4 hours.
    • An insurance relationship manager from Max Bupa will then get in touch with the policyholder to make the documentation and the claim settlement process simple.
    • The policyholder or insured person will be required to attest all documents, related forms and invoices to process the claim settlement.
