    Eligibility for Max Bupa Health Companion Family Floater

    The eligibility criteria for one to avail the Max Bupa Health Companion Family Floater are listed as follows:

    • For adults, there is no minimum or maximum age for enrollment.
    • For children, the minimum entry age is 91 days.
    • Children can be added to existing Max Bupa Health Companion Family Floater policy during renewal.
    • A maximum of 4 children can be added to a single Max Bupa Health Companion Family Floater policy.

    Features of Max Bupa Health Companion Family Floater

    The main features of the Max Bupa Health Companion Family Floater are listed as follows:

    • Based on the premium chosen by the policyholder, the sum assured is either Rs.3 lakh or Rs.4 lakh for Variant 1. For Variant 2, the sum assured options are Rs.5 lakh, Rs.7.5 lakh, Rs.10 lakh, and Rs.12.5 lakh. For Variant 3, the sum assured options are Rs.15 lakh, Rs.20 lakh, Rs.30 lakh, Rs.50 lakh, and Rs.1 crore.
    • The Max Bupa Health Companion Family Floater offers coverage for treatments in case of hospitalisation.
    • Coverage of pre-hospitalisation expenses (30 days before hospitalisation) and post-hospitalisation expenses (60 days after being discharged) - applicable for all variants.
    • All day care treatments are inclusive in the coverage - barring outpatient treatments.
    • Policyholders have the choice of topping-up their sum assured anytime in case it has been exhausted.
    • For Variant 1, policyholders are entitled to an inclusive health checkup for their family and themselves once in two years, and an annual checkup for Variant 2 and Variant 3.
    • For policyholders that opt for a two-year policy, they can avail a 12.5% discount on the premium of the second year.
    • Ambulance expenses up to Rs.3,000 are covered by the policy.
    • If the insured person is undergoing an organ transplant, the expenses to harvest the organ of the donor are taken care of by the policy.
    • The policy includes coverage of medical expenses for treatment undertaken at home - in case the insured person could not be admitted in a hospital for some reason like unavailability of beds at the hospital or if the person could not be transferred to the medical facility for treatment.. This is possible only following confirmation from the doctor and if the treatment is for a minimum of 3 consecutive days.
    • Coverage of vaccination cost in case of animal bite[s] is up to Rs.2,500 for Variant 1, Rs.5,000 for Variant 2 and Rs.7,500 for Variant 3 through reimbursement.
    • The Max Bupa Health Companion Family Floater includes hospital cash benefit up to Rs.1,000/day apart from the sum assured for Variant 1, Rs.2,000/day for variant 2 and Rs.4,000/day for variant 3. For this, the insured person has to be hospitalised for a minimum of 48 hours. This benefit can be availed for a maximum of 30 days.
    • Provided that the insured person is treated at any of the listed/network hospitals, a direct claim settlement will be taken care of by the customer care service to cover the treatment costs.
    • Unless due to an accident, no treatment will be covered for the first 30 days of the policy being issued.
    • No coverage for treatment of pre-existing diseases before completion of 48 months from the issuance of the policy for Variant 1 and 36 months for Variant 2 and Variant 3.

    Exclusions of Max Bupa Health Companion Family Floater

    Policyholders cannot avail any coverage from the Max Bupa Health Companion Family Floater based on the circumstances listed below:

    • If illness or injury is caused due to any hazardous activity.
    • If the policyholder suffers from neurodevelopmental or neurodegenerative disorders.
    • If illness or sickness is caused as a result of circumcision.
    • If the policyholder has been taking complementary or alternative types of medicine.
    • If illness or injury is caused as a result of conflict or natural disaster.
    • If illness or injury is caused due to rehabilitation.
    • If the policyholder has undergone cosmetic surgery, reconstructive surgery, or dental treatments.
    • If the patient suffers from HIV, AIDS or any sexually transmitted disease.
    • For any OPD treatment.
    • If illness or injury is caused by obesity or weight control programs.
    • Sickness arising from puberty and menopause related disorders.
    • If injury or sickness arises from any unlawful activity.
    • If the policyholder has received treatment outside India.
    • If the physician or the hospital is not recognised by Max Bupa.

    Claim Procedure

    In the case of hospitalisation, the policyholder will have to follow the steps mentioned below regarding the claim procedure. In case of planned hospitalisation (at a network hospital), the policyholder has to intimate Max Bupa at least 72 hours prior to being admitted.

    • At the network hospital insurance help desk, the policyholder has to submit his/her Max Bupa insurance card along with proof of identification - Passport, Driver’s license, Voter ID card, PAN, etc.
    • The network hospital will then submit a pre-authorisation form to Max Bupa.
    • The decision to offer coverage for hospitalisation will be passed on to the hospital by Max Bupa within 4 hours.
    • A Max Bupa relationship manager will then get in touch with the policyholder and make the documentation and the claim settlement process simple for both parties involved.
    • The policyholder will be required to attest all documents, invoices and related forms to conclude the claim settlement process.
