    To procure a driving licence to ride/drive motorised vehicles, it is necessary to first obtain a driving licence form. This form can then be submitted to an RTO along with the required documents, photographs, and fee. You will only be eligible to take the driving licence test once you submit the application form.

    How to Obtain the Driving Licence Form?

    The driving licence form is available in Regional Transport Offices (RTO) across the country. Thus, to get the driving licence form, you can visit an RTO in your jurisdiction. You can also choose to download the driving licence form through the official website of the Ministry of Road Transport and Highways (Sarathi).

    To download the driving licence form, you will need to do as follows:

    • Visit the Ministry of Road Transport and Highways (Sarathi) website. On your left, you will see a table with a number of clickable options. Here, you will have to click on ‘Download Forms’. From the drop-down list, you will need to click on ‘Print Application Form’.
    • You will then be redirected to another webpage where you will have to enter the application number of the form that you wish to download and your date of birth. The application numbers of various forms are as follows:
    • New learner’s licence: Form 2
    • New driving licence: Form 4
    • Addition of a new class of vehicle to the driving licence: Form 8
    • Renewal of driving licence: Form 9
    • Once you download the form, you can fill it up and submit it with the required documents to the RTO.

    Alternatively, you can also choose to fill the driving licence application form online. To do so, you will need to do as follows:

    • Visit the official website of the Ministry of Road Transport and Highways (Sarathi). From the options that you see on your left, you will need to click on ‘Apply Online’. Upon doing so, you will see a drop-down menu. Here, you will have to choose the type of licence that you wish to apply for.
    • Once you click on the type of licence that you require, you will be redirected to another webpage to fill the online application form. After keying-in the required details in the online application form, you will need to upload the required documents and your photographs and signature online.
    • You will then be able to book a slot for the test and pay the applicable fee.

    Basic Fields Present in a Driving Licence Form

    Certain details that you will have to enter in the driving licence application form are as follows:

    • Type of vehicle for which you wish to obtain a driving licence
    • Certain particulars such as your name, permanent address, temporary address, date of birth, educational qualification, identification marks, and blood group
    • Whether or not you previously held a driving licence
    • Whether or not you were convicted in the past
    • Whether you were previously disqualified when applying for a driving licence
    • Learner’s licence number (applicable when applying for a permanent driving licence)
    • Details of the driving certificate number
    • Written consent of the parent/guardian, if applicable
    • Details on whether the fitness certificate/application for learner’s licence has been submitted
    • Whether you are exempt from undertaking the medical test as per Rule 6 of the CMV Act, 1989
    • Whether you are exempt from undertaking the preliminary test under Rule 11(2) of the CMV Act, 1989

    List of Documents Required When Submitting the Driving Licence Form

    Certain documents that you will have to submit along with the driving licence form are as follows:

    • Address Proof
    • Three recent passport-size photographs
    • Proof of age
    • Proof of citizenship
    • Learner’s licence
    • Permanent address
    • Insurance document, registration certificate, PUC certificate, and fitness certificate (if applicable) of the vehicle that you will be driving for the test
    • Applicable fee

    You can take the driving licence test only after you fill the driving licence application form and submit it with the above-mentioned documents. If you successfully clear the test, you will receive the driving licence.
